Really depends on your preferences and budget. In wall are more expensive and yes if something does go wrong it can mean opening up wall. How it looks is up to your preference
We spent a year in a place with a cistern in the wall toilet. Absolutely fine until something goes wrong -which it did. No easy fix of taking off the the lid and getting it to flush. Wall/tiles have to be removed. We would never install one. Back to wall gets my vote
I have an in-wall cistern. Wouldn’t have another due to the difficulty in doing even minor maintenance with seals. Back to the wall, and close couple suites have come a long way in looks and compactness so they’d get my vote.
